JOB ANNOUNCEMENT
Title: Resident Monitor Division: HOPICS / Pathway Home
FLSA: Non-Exempt, Full-time Supervisor: Program Supervisor
Pay Range or Rate: $22.50hr to $24.00 per hour. Revised: 03/12/2024.
Summary: Under the direction of the Pathway Home Program Manager/Supervisor the Resident Monitor will be responsible for providing client care, provide service coordination, and facility safety plans for interim housing clients. The resident monitor will provide client assistance to clients daily.
Essential Functions: ● Must know, understand, and be able to articulate the mission, vision, and core values of HOPICS
● Work collaboratively with Program Managers, Supervisors and Direct Service Staff, and security
● Meet with management staff on daily to weekly basis to discuss findings
● Provide light case management, developing individual housing service plans, making linkages, and securing needed services for participants.
● Obtain and maintain the working knowledge of all LAHSA Funded Programs and their eligibility
criteria
● Provide support to the program staff in formulation of program procedures and timely dissemination to staff
● Meet with participants regularly or as needed to discuss needs, progress, and barriers.
● Assist in developing and implementing client life-skills needs
● Coordinate with other project staff and partners to provide housing placement services and supportive services to facilitate client’s successful stabilization in permanent housing and coordinate services to foster housing stability.
● Coordinate and Maintain calendar for Case Manager and housing navigators, as needed
● Provide administrative support/coordination with special projects as necessary
● He/She will assist clients with identifying employment opportunities, becoming employment readiness, training and placement as needed or requested.
● Ensure that incident reports are submitted and reported within 24hrs of a shift.
● Complete daily room checks, and provide shift pass down clients information after each shift
● Must travel between HOPICS / Pathway Home program locations to support case managers, housing navigators, and other monitors within the programs.
● Understand and accurately articulate the HUD definition of homelessness and chronically homeless
● Attend and record all program, department, funding specific and other associated meetings, and
training as required or needed.
● Some evenings and weekends required
● Perform any other appropriate responsibilities as assigned by the Program Supervisor and or Program Manager.
● Ability to understand and carry out oral and written direction
● Maintain files/records on client services in compliance with HIPAA, 42 CFR Part 2 and other
funding requirements for audit purposes.
● Maintain appropriate boundaries with staff and partners; and adhere to SSG’s Code of Ethics and
HOPICS’ Core Values.
● Represent the Agency in a professional manner at meetings and community events
● Regular attendance required.
● Other duties as needed
Case Management Support: ● Must be familiar with and able to provide support to case managers on program related documents, including but not limited to individual service plans, psycho-social assessments, Housing for Health documentation, and internal HOPICS documents
● Provide case management support to all LAHSA Funded programs, as needed
● Work with direct service staff to ensure maintenance of and accurate documentation in client files for service coordination/case management, housing placement, move in assistance, community re-
integration and any other additional services provided.
Secondary Functions: Work on special projects as assigned.
Perform other duties as assigned by Program Manager, Program Supervisor and/or Associate Director.
Supervisory Responsibilities This position does not have any supervisory responsibilities.
Minimum Qualifications - Knowledge, Skills, and Abilities Required Associate degree in systems, Social Services, or other administrative or business sector from an accredited or state-approved college or university, with a minimum of 1-2 years of job-related experience, OR a commensurate combination of relevant education. If in recovery, a minimum of three (3) years of being drug and alcohol-free is required. High Proficiency in HMIS, Microsoft Word, Excel, Access, and other database programs. Accurate and concise documentation. Neat and professional appearance. Attention to detail and punctuality are required.
MANDATORY Qualifications: Ability to work with clients from diverse cultural, ethnic, and socio-economic backgrounds. Ability to communicate effectively, both written and orally. A valid California Driver’s License and proof of insurance are required, and reliable transportation is required. Verification of Employment Eligibility and Background Check is required. TB test required (Note: Results may not be more than (3) months before or (7) days after the date of hire and renewed annually thereafter.) CPR and First Aid certifications are required within 30 days of employment with the company.
Environmental Conditions (Working Conditions) May be exposed to highly aggressive clients. Must communicate with many sources including public paying agencies, courts, and outside community agencies. Local automobile travel is required. Some evenings and weekends may be required. Sometimes noisy, loud, and disruptive clients.
Physical Requirements The Program Manager typically spends time sitting, standing, walking, driving, and carrying (max. 25 lbs.), listening, speaking.
Mental Requirements This position will require the individual to be able to handle any/all the following: constant distractions, interruptions, uncontrollable changes in priorities/work schedules: and be able to process information, think, and conceptualize.
